APPOINTMENTS
Please let our receptionists know the general nature of your needs so she can schedule an appropriate amount of time. For urgent problems, please call as early as possible so that we can see you the same day.

The quickest way to make an appointment is to call us directly. However, if you prefer the convenience of the Internet, simply complete the form and we'll call you back.

The First Visit
New patients can make their first visit easier by printing and completing new patient forms before they come into the office. If you have completed the forms, you may either mail them to our office or bring them with you. Please arrive at least 15 minutes prior to your appointment time so we can finish up the paperwork. Once you have not completed the forms, please arrive at least 20 minutes prior to the appointment time. Also, don't forget to bring the most current copy of your insurance card and required co-payment.
Delays
We value your time and do not want to keep you waiting. Occasionally, we are delayed or must reschedule due to a delivery or unexpected event with another patient. If you are delayed well past your scheduled appointment time, we will make every effort to meet your needs, but your appointment may have to be rescheduled. We appreciate your understanding.
Cancellations
If unable to keep an appointment, please notify us as soon as possible. This courtesy allows us to schedule another patient in your place.
Inclement Weather
To support the safety of our patients and staff we may close the office or limit office hours if adverse weather conditions arise. Please call prior to your appointment time when such circumstances occur.
